Job Description
The clinical transplant coordinator oversees pre transplant evaluation of patients and make sure that it is complete in the appropriate time
Fixing appointment for the patients with the consultants and getting other investigations done before surgery
The TC is responsible for collating information from multidisciplinary consultants
The clinical transplant coordinator will ensure that each transplant team makes a list of patients assessed by them as needing organ transplant
The TC plays the role of the first point of contact for transplant patients
The TC will organize meetings- listing meeting on ongoing management of patients
The TC will make sure that the decisions taken on the listing meeting have been implemented on time
The TC coordinates registration of patients with Jeevasarthakathe for cadaveric donor transplantation
The TC guides the patient and family for the legal documents for live & cadaver donor transplantation
Maintaining waiting list and patient follow up
Counselling of patients on waiting list for deceased donor transplantation
The TC will keep in touch with the ICUs in the hospital and when a brain death is suspected, he/she will make sure that the certification of brain death is done as per the legal requirements
Provide Grief counselling to relatives and request for organ donation and alert Jeevasarthakathe if donation is feasible
TC ensures deceased donor data form is filled and updated to the transplant team for donor assessment
The TC will make sure that the donor family is at most comfort at the hospital
Liaise with Jeevasarthakathe for the entire coordination of organ donation
Alert the organ retrieval team for the possible organs to be retrieved
Alert the Lab heads, operation theatre and the concerned departments for the transplant
Assist the transplant team in activating the recipient patient
Send blood sample or make the arrangement for DSA cross matching of kidney transplant patients
Ensure donor blood investigations are done and informed to the transplant team
Ensure donor blood sample has been stored to perform investigations in future
Ensure requirements of the transplant/retrieval team and ZCCK are provided
Ensure donor and recipient families are informed of the events during the process
Educate donor, recipient and family about the living donor transplant process during initial assessment and whenever needed
Guide the family about the necessary documents required to get approval from Competent Authority or Authorization Committee
Make sure Informed consent procedure has been video recorded
Alert all concerned departments and staff of the scheduled organ transplant with details
Make sure all investigation reports and films are collected, filed and available on the transplant day
Coordinate with doctors for the admission
Coordinate with ICU, OT, Lab and patient family during operation
Maintain patient’s records and files – pre transplant work up records, post - transplant follow up records etc.
