Job Description – Client Care Coordinator Department: Social Work Reporting to: Case Manager Work Location: Sowripalayam QUALIFICATION REQUIRED: • M.S.W. (Medical & Psychiatry) • Experience in rehabilitation, mental health, or hospital settings is preferred. ROLE SUMMARY: The Social Worker plays a supportive and service-oriented role in ensuring smooth client functioning and interdepartmental coordination. This role bridges the client experience with operational needs, ensuring the seamless delivery of therapeutic services, supervision of privileges, and coordination of daily routines and special requirements. The Social Worker must be observant, proactive, and responsive to both routine and emergent client concerns, embodying Abhasa’s core values of Belongingness, Compassion, Expertise, and a Tailor-Made Approach to care. KEY RESPONSIBILITIES: Client Support & Monitoring • Conduct daily rounds to monitor client welfare and observe concerns. • Address and report client emotional disturbances or behavioural deviations. • Supervise phone and laptop use of clients, ensuring safe and healthy digital habits. • Assist clients during mealtimes (Dining Hall Duty) ensuring appropriate behaviour and engagement. Group Activity Coordination • Coordinate and assist in the execution of scheduled group activities including JFT, FFT, Psychoeducation, Art Therapy, Movement Therapy, Alchemy, and others. • Support the facilitation of Saturday Special Activities, helping ensure client participation and safety. • Assist in the Chai Corner Privilege process, encouraging client engagement in informal therapeutic spaces. Outing & External Support • Escort and support clients during outing programs or external consultations. • Ensure safety and therapeutic continuity during all outside movement. Interdepartmental Collaboration • Attend and actively participate in interdepartmental meetings. • Serve as a liaison between clinical, operations, housekeeping, and therapy teams to ensure timely execution of client needs. • Facilitate purchase orders for client-related materials or therapy requirements. Documentation & Reporting • Maintain daily logs related to client behaviour, privileges, and participation. • Report incidents and concerns promptly to the Case Manager. • Ensure proper documentation of POs, external visits, and tech use monitoring.
